How to apply

1The Customer is required to have an account at Bank Sinarmas as the debit source account.
2The Customer is required to fill in and sign SPAJ (Life Insurance Application Letter).
3The Customer is required to attach a copy of ID card.
4The Customer is required to attach the evidence of transfer.
